Failing to keep your appointment or to give the office at least 24 hours notice of being unable to attend your appointment will result in various fees.

There is an additional $35 dollar fee for completion of forms for use outside of the patient's medical record, including but not limited to:

  • Day Care/Foster Care

  • Pre-Employment/Employment

  • Disability

  • Life Insurance

  • Family Medical Leave
